Holly Creek is Proud to Introduce a New Medicare Advantage Plan & Enhanced Benefits to our Existing Plans that Elevate Care Coordination

Holly Creek offers residents friendship, freedom, and opportunity. We’re always on the lookout for new ways to help residents and their families. Since 2021, our relationships with Redwood Health Partners and Perennial Advantage, our Medicare Advantage plan built just for our residents, have provided each older adult access to a new approach of transformative medical services.

For 2023, we have created a brand new plan just for older adults in our Residential Living neighborhood and enhanced the benefits of both existing plans available to Holly Creek residents.

• Perennial Advantage Freedom – is available to all residents who do not qualify for one of our other plans designed for those with special needs.
• Perennial Advantage Strive – is available to resident Medicare beneficiaries who meet the requirements for long-term care services.
• Perennial Advantage Concierge – is available to resident Medicare beneficiaries with a diagnosis of a chronic condition like congestive heart failure, diabetes, or heart disease.

Perennial is a customized Medicare Advantage Plan partially owned by our parent-company, Christian Living Communities, for the sole purpose of serving residents’ specific needs. Their mission is to provide better outcomes, increased satisfaction, and lower health costs to our residents. It all starts by bringing attention to the preventive lifestyle and chronic care management within our community. Redwood Health Partners gives Holly Creek residents the convenience of having expert medical care right down the hall or in the privacy of their apartment or suite. A dedicated nurse practitioner hosts regular in-community clinic hours. From regular checkups to managing symptoms of an illness, residents have peace of mind knowing that expert medical support is always within the community. Our dedicated medical concierge is available to coordinate medical visits, lab results, and preventive care.
