Alzheimer’s and Dementia Care
For those living with memory loss, dementia, or Alzheimer’s, we offer a secure and comfortable memory care community designed to maintain dignity, and strengthen and celebrate life skills. Our 12-suite memory care neighborhood includes discrete security and a private garden to allow for safe enjoyment of the outdoors. Each suite is private and includes 500 square feet of living space, a living area, a kitchenette, and a full bathroom.
Through Rhythms Dementia Services, we offer meaningful memory care programming and services that focus on the strengths of each individual, rather than any limitations.
About Our Memory Support Neighborhood
Our Dementia Care Philosophy
Our holistic approach to memory care – Rhythms Dementia Services – adapts to each person’s natural rhythm of life, to help create a supportive, positive environment.
Rhythms Dementia Services was developed to provide customized, empowering care for those who are experiencing physical and cognitive changes. We understand that people living with Alzheimer’s or dementia experience the world differently, and are focused on supporting residents to safely and successfully live a meaningful life.
Our specially trained associates strive to build strong relationships with each resident and encourage connection and interaction. Additionally, all care partners receive at least 8 hours of annual training in providing memory care for those with Alzheimer’s or dementia.

Meaningful Programming
Holly Creek is the vibrant community it is because of the purpose-driven, passionate residents who call it home. We honor residents’ rhythms of life and strive to engage their passions through meaningful connections and enriching programming. Our life enrichment program is built around social connection, intellectual engagement, purposeful living, physical health and spiritual growth. We respect residents’ autonomy and choice by developing activities, classes, and events with residents.
We have incorporated the The Eden Alternative approach into community life, affirming that care is a collaborative partnership. Holly Creek residents and team members take part in immersive Eden Alternative training aimed at building partnerships between older adults and care partners focused on continued growth.
